• Responsible for the daily operation of the dementia-specific, social model daycare program including, but not limited to, daily activities, supervision and scheduling of volunteers, safety of participants, attendance records, and client files. • Recruit, train, supervise volunteers and maintain volunteer personnel files. • Exercise independent judgment to apply expertise in conducting initial intakes and evaluations. • Complete timely and accurate client notes, charts and other documentation related to both program participants and family caregivers, including data entry • Ensure that all medical histories, medical directives, emergency contact information and emergency response plans are in place, as well as other records required by licensing authorities. • Connect clients to resources in the community to provide advocacy and referrals. Acquire ongoing knowledge of local services and collaborate with other community agencies as appropriate. • Foster peer support and coordinate information and care requirements with other direct care staff • Conduct individual counseling sessions and a variety of support groups. • Supervise college/master’s level interns and serve as liaison to their college supervisor(s). • Maintain client confidentiality per Memory Matters policy and HIPPA regulations. • Participate in regular staff meetings, staff training programs, supervisory sessions. • Accept the responsibility for positive team relationships as requested. • Serve and protect Memory Matters by adhering to agency policies, procedures, professional standards, as well as federal, state and local requirements. • Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the-art practices; serving as a Memory Matters ambassador in the community. • Participate (as needed) in Memory Matters special events and educational presentations. • Conduct presentations on Memory Matters services as required.


Full-time care management position; minimum requirement is a bachelor’s degree in human services or related field. Experience working with elders preferred; experience with Alzheimer’s and all forms of dementia preferred. Knowledge of case management systems and ability to work sensitively with a variety of cultures and lifestyles required. Ability to function autonomously and as a member of an interdisciplinary team. Excellent oral and written communication skills, problem solving and conflict resolution skills. Familiarity with local/regional/state and federal health and human services resources. Proficient computer skills and ability to use word processing, data base and other applications. Knowledge and experience regarding grief, loss and coping skills.
