Coastal Medical Staffing

LPNs needed for nursing home setting in Savannah, GA and Hilton Head/Bluffton, SC. Perform direct resident care in accordance with nursing standards of care utilizing the principles of nursing and basic physical assessment skills in the development, documentation, and implementation of individualized, interdisciplinary care plans to ensure the needs of the residents are met. Participate in developing, implementing, and evaluating resident's plan of care; and conduct resident care conferences, as needed. Assess residents, initiate physician orders, administer medication, and complete procedures and treatments in accordance with resident needs, physician orders, and procedures. Accurately record resident care and observations on their medical record. Work cooperatively and respectfully with team members, residents, residents' families, visitors, and volunteers to create a positive and pleasant "home" environment within their neighborhood. Complete nursing rounds, provide follow-up supervision, assist/consult with nursing/medical staff, inform team members of the care needs of the residents, conduct change-of-shift reports, and assign team members based upon their level of training. Serve as a resource person to others, promote a positive work environment, and assist in resolving disputes and/or redirecting negative behaviors. Maintain confidentiality of department, resident, employee information, communication, and documentation as well as compliance with Health Insurance Portability and Accountability Act (HIPAA) regulations. Inform a Nurse Manager, House, Supervisor, Registered Nurse, resident's Guardian, and resident's Physician of any change in residents' condition in a timely manner; initiate follow-up to promote resident safety/care; and immediately report any suspicion/allegation of resident abuse/neglect to supervisor. Assist in the admission, transfer, and dismissal of residents; complete all related documentation; and assist with the care of the residents. Treat residents, coworkers, and the public with dignity and respect at all times. Serve as a mentor/preceptor to coworkers and agency/outside personnel; follow-up to ensure that they function within the scope of practice; and help others learn by teaching and being a positive role model. Encourage, empower, and support resident's, whenever possible, in maintain or regaining their independence, accommodating their individual choices, assisting them in directing their own daily life, and in helping residents maintain their own personal belongings according to their wishes. Investigate complaints and/or suggestions from residents, coworkers, or the public; and take steps to remedy problems. Take steps to promote and maintain a clean and safe work environment, and ensure that malfunctioning equipment is immediately removed from service, and tagged for repairs.


Must be licensed in the state.
